OFFICIAL RULES FOR THE COSE HOME BUSINESS NETWORK
“MY UGLY HOME OFFICE” CONTEST
  1. NO PURCHASE NECESSARY. YOU MUST BE 18 YEARS OF AGE OR OLDER AND A LEGAL RESIDENT OF OHIO FOR THIS PROMOTION. YOU MUST BE A SMALL BUSINESS OWNER (50 EMPLOYEES OR LESS) WHOSE DAY-TO-DAY BUSINESS OPERATIONS AND PRIMARY OFFICE ARE BASED OUT OF THEIR HOME, WHO HAS BEEN IN BUSINESS FOR TWO YEARS AS OF SEPTEMBER 1, 2008. DATE OF BUSINESS OWNERSHIP WILL BE VERIFIED VIA BUSINESS OWNER’S TAX IDENTIFICATION NUMBER. HOME-OFFICE MUST BE LOCATED WITHIN OHIO. VOID WHERE PROHIBITED BY LAW. The space to be enhanced may be no larger than 8’ x 12’. A purchase of any of COSE’s products or services will not increase your chances of winning. By submitting your entry, you accept and agree to these rules and procedures of the Sponsor.

  2. METHODS OF ENTRY: No purchase necessary. To enter the COSE Home Business Network “My Ugly Home Office” Contest (the “Contest”), visit http://www.myuglyhomeoffice.com and fill out the online entry form, commencing on August 20, 2008. Entrants will provide on the online form their name, business name, business tax identification number, street address, city, state, zip code, County, email address, daytime telephone number with area code, business website, how they heard about the contest, whether they are a COSE member, number of employees, number of years in business, a brief description and mission of business, a 150 word essay on why they need or deserve a home office makeover, at least one (1) and no more than three (3) photos of their home office space and (1) photo of themselves. Entrants are limited to business owners.

    The Sponsor is not responsible for entry submissions received after deadline and/or with incomplete information. No mass entries will be accepted. Entries limited to one entry per business owner. All entries become the exclusive property of the Sponsor and will not be returned or acknowledged. Entry information will be used only by the Sponsor, its employees, its subsidiaries, or agents; it will not be shared with other parties. The Sponsor assumes no responsibility for lost, late, misdirected, illegible or mutilated entries. The Sponsor reserves the right to suspend or terminate this Contest without notice if the Sponsor determines, at the Sponsor’s sole discretion, that there is suspected tampering with the Contest entry materials or non-compliance with the rules has occurred. In the event that the Contest is terminated, the Sponsor will conduct a random drawing to award the prize using all eligible entries received as of the termination date. If the Contest is terminated due to any of the above reasons prior to its expiration date, notice will be posted at http://www.myuglyhomeoffice.com. CAUTION: ANY ATTEMPT BY AN INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS CONTEST MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO DISQUALIFY ENTRANT AND SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW.

    By entering this Contest, entrants agree to be bound by these Official Rules and the decisions of the judges, which shall be final, binding and conclusive on all matters relating to this Contest. Contest starts at 12:01:01 a.m. ET on August 20, 2008 and ends at 4:59:59 p.m. ET on September 30, 2008. Entries must be completed online by 4:59:59 p.m. ET on September 30, 2008.

  3. PRIZE/ODDS: GRAND PRIZE (1): One Grand Prize winner will receive a retail value of $3,000.00 in office furniture from Today’s Business Products for their personal home-office and a one (1) year renewal or membership to COSE. The space to be enhanced may be no larger than 8’ x 12’. If winner is unable to participate in the makeover on dates specified and agreed to, prize will be forfeited and another winner may be chosen. No cash substitutions for prizes will be permitted and prizes are non-refundable and non-transferable except at sole discretion of the Sponsor. The Sponsor reserves the right to substitute prize of equal or greater value. Federal, state and local taxes are the sole responsibility of the winner. Actual odds of winning depend on the number of valid entries received. All federal, state and local taxes, if any on the Grand Prize, are the sole responsibility of the winner. The prize will be awarded by the Sponsor.

  4. JUDGING: Winner will be selected by a panel of judges on or about September 29, 2008 from among all valid entries received online. Winner will be contacted by telephone on or about September 30, 2008 at the winner’s business phone number as provided. Failure to reach winner by phone will result in disqualification and an alternate winner will be selected.

  5. ELIGIBILITY: NO PURCHASE NECESSARY TO ENTER OR WIN. Contest open only to legal residents of Ohio who are 18 years of age or older. You must be a small business owner whose primary office is their home, who has been in business for two years as of September 1, 2008. Date of business ownership will be verified via business owner’s tax identification number. Home-office must be located within Ohio. Employees of the Sponsor, and its agencies, parents, subsidiaries, affiliates, vendors, wholesalers or retailers, or members of their immediate families or households, are not eligible to participate. Contest void where prohibited by law. Federal, state and local laws and regulations apply.

  6. AFFIDAVIT OF ELIGIBILITY: Grand Prize winner will be required to complete and return an Affidavit of Eligibility/Liability/Release and, where lawful, a Publicity Release. Affidavit of Eligibility/Liability/and Publicity Release must be returned within three (3) days of notification or the First Prize will be forfeited and an alternate winner will be selected. Acceptance of this prize constitutes permission for the Sponsor and its advertising and promotional agencies to use the winner’s name and likenesses (including photography or video footage of winner and winner’s home and business property) for advertising and promotional purposes without additional compensation or notice unless prohibited by law.

  7. RELEASE OF LIABILITY AND PUBLICITY: The three Contest finalists and the ultimate Grand Prize winner consent to the use of his/her name, photographs or likeness for publicity or advertising purposes without compensation, where permitted by law. All entrants release the Sponsor, and each of its parents, affiliates, subsidiaries, officers, directors, shareholders, agents, employees and all others associated with the development and execution of this Contest, from any and all liability with respect to, or in any way arising from, this Contest and/or acceptance or use of the prize, including liability for personal injury, damages, death or monetary loss. The Sponsor and its agents are not responsible for telephone/computer malfunctions of any kind or failed, incorrect, incomplete, inaccurate, garbled or delayed communications caused by the user, or by any of the equipment or programming associated with or utilized in this Contest, or by any human or other errors which may occur in connection with this Contest. Contest, entrant further agrees that: a.) under no circumstances will entrant be permitted to obtain awards for, and entrant hereby waives all rights to claim, punitive, incidental, consequential, or any other damages, other than for actual out-of-pocket expenses; b.) all causes of action arising out of or connected with this contest, or any prizes awarded, shall be resolved individually, without resort to any form of class action; and c.) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorneys fees. Sponsor reserves the right to modify prize award procedures. The laws of the State of Ohio without applying its conflict of law provisions shall govern all disputes concerning this contest.

  8.
